Welcome, to our established and new patients. Our doctors and medical staff are all excited to spend quality time getting to know you and your medical needs. Our established patients may contact the office to book their next appointment. We require that ALL patients have blood work done a minimum of 1 week prior to their appointment. If you have recently had blood work done from another provider, please bring the results with you as well as your updated medication list.
New Patient Forms
For our new patients, forms are available on our website or we invite you to stop into either one of our two practice locations, Delray Beach or Boynton Beach, to obtain a hard copy. We recommend completing all paperwork prior to your appointment. Please bring your medication bottles or medication list with dosages and frequency, insurance card, and photo ID card. New patients must arrive 30 minutes prior to their appointment time. We ask that you come hydrated as we will be asking for a urine sample.

It is advisable that you contact your insurance company by calling the customer service number on the back of your card to verify that we are in-network with your plan prior to making an appointment.
